JWCH HIV Case management services are provided to the disenfranchised HIV positive homeless persons who have a continual and increasing need for assistance. These services include: Complete psychosocial assessment; individual care plan development; appropriate referrals to housing, medical, mental health and drug treatment; meals and nutritional supplementation; and hygiene items, clothing and HIV education groups; Clients in need of services are identified through a variety of community sources including:
- JWCH Institute's Skid Row Medical Outreach Team
- Current or past clients of the program,
- other community based organizations,
- AIDS service organizations and
- Health facilities such as clinics and area hospitals.

The Transitional Case Management Program provides services to incarcerated individuals who are living with HIV and are transiting back to the community. These services include: Complete psychosocial assessment; individual care plan development; appropriate referrals to housing, community case management, medical, mental health and drug treatment.
