JWCH HIV Case management services are provided to the disenfranchised HIV positive homeless persons who have a continual and increasing need for assistance. These services include: Complete psychosocial assessment; individual care plan development; appropriate referrals to housing, mental health and drug treatment; meals and nutritional supplementation; and hygiene items and clothing. Clients in need of services are identified through a variety of community sources including:

• JWCH Institute's Skid Row Medical Outreach Team
• current or past clients of the program,
• other community based organizations,
• AIDS service organizations and
• health facilities such as clinics and area hospitals.
