HIV/AIDS Services

 
HIV Case Management

JWCH HIV Case management services are provided to the disenfranchised HIV positive homeless persons who have a continual and increasing need for assistance. These services include: Complete psychosocial assessment; individual care plan development; appropriate referrals to housing, medical, mental health and drug treatment; meals and nutritional supplementation; and hygiene items, clothing and  HIV education groups; Clients in need of services are identified through a variety of community sources including:

  • JWCH Institute's Skid Row Medical Outreach Team
  • Current or past clients of the program,
  • other community based organizations,
  • AIDS service organizations and
  • Health facilities such as clinics and area hospitals.

Transitional Case Management Program

The Transitional Case Management Program provides services to incarcerated individuals who are living with HIV and are transiting back to the community. These services include: Complete psychosocial assessment; individual care plan development; appropriate referrals to housing, community case management, medical, mental health and drug treatment.

ALPHA Project

The JWCH Alpha Project is an adherence case management program that targets HIV positive monolingual Spanish speaking clients. These services include: Complete psychosocial assessment; individual care plan development; Spanish speaking support groups; appropriate referrals to housing, medical, mental health and drug treatment; meals and nutritional supplementation; and hygiene items and clothing.

HIV/AIDS Residential Housing

JWCH provides emergency and transitional housing services to people living with HIV/AIDS with 15 beds nightly located at the Weingart Center. These services include: Complete psychosocial assessment; individual care plan development; appropriate referrals to housing, medical, mental health and drug treatment; meals and nutritional supplementation; and hygiene items and clothing.

Young Men's Connection Program

The Young Men’s Connection (YMC) is dedicated to reducing the spread of HIV and to reduce/eliminate substance use among young men who have sex with men and who are African-American or Latino in the greater Los Angeles area.
